Effective: September 2026. This is a summary; patients receive the full notice at intake.

How your information may be used

Ferncrest Psychiatry may use and share your protected health information to provide treatment, obtain payment, and run practice operations — for example, coordinating with your therapist or billing your insurer. Most other uses, including marketing and the sale of information, require your written authorization.

Special protections

Psychotherapy notes, substance-use records, and certain other sensitive information receive additional protection under federal and state law and are shared only as the law permits or with your authorization.

Your rights

You have the right to see and request a copy of your records, request corrections, request confidential communications, request limits on certain uses, and receive an accounting of disclosures. Complaints

If you believe your privacy rights were violated, you may complain to the practice or to the U.S. Department of Health and Human Services. You will never be penalized for filing a complaint.
